CHRONIC CONDITIONS

Chronic Conditions

Chronic health conditions can impact your quality of life, making it important to manage them effectively. Common chronic conditions include:

  • High blood pressure
  • Ischemic heart disease and heart failure
  • Diabetes
  • Asthma
  • Obesity
  • Chronic obstructive pulmonary disease (COPD)
  • Allergies, eczema, and hay fever
  • Osteoarthritis
  • Mental illness and dementia
  • Systemic lupus
  • Polycystic kidney disease and chronic renal failure
  • Osteoporosis
  • Recurrent kidney stones and gallstones
  • Cancers
  • Chronic pain

It’s important to note many of these conditions may not show any symptoms, and are often detected during routine health screenings. Effective management of chronic conditions usually requires ongoing monitoring through regular appointments and may include blood tests, x-rays, input from specialists or allied health professionals and balancing multiple medications or therapies.

Doctors consulting at Morningside General Practice Clinic are experts in managing and coordinating continuity of care for patients with chronic conditions. This includes hospital in/outpatient referrals, private health interventions and multiple specialist referrals. We have dedicated Nurses to assist the Doctor in putting together comprehensive care plans to optimise your health and well-being.

Organising your care plan requires a specialised appointment with both your Doctor and the Chronic Disease Nurse, as they take their time to listen to your concerns and explain your condition in detail, empowering you to maintain the best quality of life possible.
